Each participant in this Consortium Institute of Management and Business Analysis (CIMBA) program is required to have suitable health/accident insurance coverage. The information requested on this medical information form is intended to help inform CIMBA staff of any pre-existing medical conditions and to help determine if consultation with your physician is recommended prior to participating in this training. If you have a pre-existing medical condition, participation in some of the more strenuous activities may not be recommended. This information will be kept in strict confidence by CIMBA and will only be shared with your express, written consent.
